Step 1.Make sure you have the correct agency 

The City of Alameda does NOT accept service of claims on behalf of (please note this is not a complete list):

  • Alameda County
  • Alameda Housing Authority
  • Alameda Unified School District
  • Alameda Sheriff's Department
  • Alameda Hospital
  • AC Transit
  • Highway Patrol

Step 2.Check your eligibility

It's important to file claims in a timely manner. Keep in mind:

  • Claims for personal injury, wrongful death or property damage must be filed with the City of Alameda no later than six months after the incident date.
  • Claims relating to any cause of action other than personal injury, wrongful death or property damage must be filed with the City of Alameda no later than one year after the incident date.

Step 3. Gather the necessary information 

You will need your contact and certain personal information, and information relating to the claim e.g. when and where did the incident happen, what happened, why is it the City’s fault, description of damage or loss and total amount claimed.  Be as detailed as possible.

Step 4. Complete a claim form

When filling out the form keep in mind:

  • All fields are mandatory. 
  • Be as specific as possible. 
  • Failure to complete a field, or vague or incomplete responses may lead to a delay or denial of your claim.

Step 5.Submit your claim form to the City

Once you complete the form, sign it, and attach any supporting documents. 

Claim forms need to be filed with the City Clerk either in person, Monday – Thursday, 8:00 am – 6:00 pm, or by mailing the form and any attachments to:  City Clerk, City of Alameda, 2263 Santa Clara Ave., Suite 380, Alameda, CA 94501.

Step 6. We will process your claim

Once a claim is filed with the City it will be evaluated by the Risk Manager and then forwarded to our Third Party Administrator (“TPA”). You will receive a letter from the TPA after the claim is received.  Additionally, the TPA will notify you if they need further information or documentation.
