Digital COVID-19 Vaccine Record

If you are asked for your vaccine record, you can show your physical vaccine card or you can receive a digital copy of your vaccine record from the State of California.

  • You will be asked to enter your first name, last name, date of birth, and the cell phone or email address associated with your vaccine record. Then you'll create a 4-digit pin number. You will be sent a link (via text or email) to retrieve your digital vaccine card and will be asked for the pin number you created.
  • If you have an iPhone: You can save your digital vaccine record by taking a screenshot and saving it to your camera roll. You can also save the screenshot to your iPhone files.
  • If you have an Android phone: You can save your digital vaccine record on Google Pay or you can save by taking a screenshot and saving it to your camera roll.
